Please complete the form below to register to attend the Firefighter Day Camp.

Please note - please fill out a separate form per each participant.

All registered campers will receive a free camp T-shirt and lunch.

You will also receive more information approximately one week before camp starts.

There are two section to this form the first part is about the Parent/Guardian and the second part is about the Participant/Camper.

CONSENT, RELEASE OF LIABILITY, AND INDEMNIFICATION AGREEMENT In consideration of the City of Carmel, Indiana (hereinafter referred to as “City”) allowing my child/children to participate in the Carmel Fire Department’s “Firefighter For A Day Camp” (hereinafter referred to as “Camp”), I hereby RELEASE, INDEMNIFY AND DISCHARGE the City, and the City’s departments, agencies, officers, agents, sponsors, officials, attorneys, volunteers, and employees from and against all claims, losses, liabilities, expenses (including attorney fees), costs and damages, either direct or consequential, however occurring, from and for any loss, injury (including death) and/or damage of or to any person, including my child/children, or property resulting from or claimed to be a result of any conduct or act taken or omitted by or to my child/children while or as a result of his/her participation in the Camp, whether due in whole or in part to negligent acts or omissions of the City, or the City’s departments, agencies, officers, agents, sponsors, officials, attorneys, volunteers, or employees. I understand and agree that the nature of the Camp may involve a degree of risk from a variety of sources which could result in serious bodily injury to my child/children (including death) and/or damage to my child/children’s property, and that, being fully advised and aware of the same, I assume and accept full responsibility for any risk that may result from my child/children’s participation in the Camp. I further acknowledge that I am fully aware of the nature and hazards of participation in safety training activities such as those offered in the camp, which may include, but are not limited to, running on an obstacle course, climbing ladders, crawling low under simulated fog smoke, rolling on mats, holding a pressurized fire hose, and other activities that would involve running and/or crawling in a variety of potentially dangerous environments. By allowing my child/children to attend the camp I acknowledge and consent to the use of any photographic images of the Camp attendees, instructors and volunteers that may be captured during the Camp, and further understand that photographic images and video and audio recordings may be used as promotional materials by the City. I have read the above “CONSENT, RELEASE OF LIABILITY, AND INDEMNIFICATION AGREEMENT,” and understand the potential hazards of my child’s/children’s participation in the Camp. On behalf of myself, my spouse, if any, and above-named child/children, and in exchange for the participation of my child/children in the camp, I hereby accept all the risks and responsibilities, and release the City from any and all liabilities, as stated above.
